Why Shoulder Surgery Isn’t Always the Answer: What Research and Clinical Guidelines Say—And What to Do Instead 

 

Shoulder pain is one of the most common musculoskeletal complaints in the world. From rotator cuff tears to impingement and frozen shoulder, millions of people experience discomfort, stiffness, and loss of function each year. Research and Clinical Practice Guidelines don’t support surgery as the first line of treatment for most shoulder conditions.

  • The American Physical Therapy Association (APTA) CPG for subacromial pain syndrome recommends exercise and manual therapy as first-line treatment. 
  • Surgery (like subacromial decompression) shows no meaningful benefit over physical therapy in controlled trials. 
  1. Adhesive Capsulitis (Frozen Shoulder) 
  • Guidelines emphasize graded movement therapy and education, not surgery. 
  • Surgical capsular release carries risks and often requires long rehab—with no better outcomes than well-structured physical therapy. 

 

In short: Surgery is expensive, invasive, and often no more effective than movement-based therapy—especially when the root cause of pain is mechanical or functional.

Surgery may treat the symptoms—but it doesn’t always address why the shoulder broke down in the first place.

overreliance on the shoulder due to hip, foot, or ribcage dysfunction
