The 3 main diagnoses a doctor might give as a cause of shoulder pain:
Impingement Syndrome- pain along front and top of the shoulder due to decreased space within the shoulder.

Labral Tears- The cartilage ring around the shoulder socket gets torn and impedes shoulder motion, causing pain, clicking, and popping.
What many physicians don’t realize is that these diagnoses don’t cause shoulder pain. Instead, there is a musculoskeletal dysfunction in or around the shoulder that is causing the pain, inflammation, and, in some cases, tears. The good news is that these dysfunctions can be treated with proper physical therapy.

The Shoulder Blade Lacks Mobility
It is very, very common for tight upper back muscles to stop the four shoulder joints from moving in sync. This is by far the most common reason why shoulders are painful and inflamed.
The Shoulder Joint Lacks Stability
The muscles around the shoulder joint are not providing enough strength and stability allowing the head of the humerus to move around too much, which can create impingement and inflammation in the soft tissues of the shoulder.
Poor Upper Spine Mobility
The shoulder blade is a free-floating bone that moves along the rib cage. The ribs are attached to the upper spine. So ultimately it is crucial for the upper spine to move properly. When this important musculoskeletal chain reaction isn’t working properly, shoulder pain is the result.
